Name Change in SLATE

Overview

Please note that SPARK only receives name updates through the OTR and HR. All updates, including changes to both Preferred and Legal names, must adhere to the OTR’s established Name Change Process.

In Slate, the team will update the preferred name based on OTR confirmation but legal name changes are updated by an integration process.

Please find below the process for changing your name in the SLATE portal.

For Students: To submit a name change request, please follow the guidelines outlined on the OTR process page.

For Staff: To submit a staff name change, please contact Human Resources.

In the case of a name, email, or username change, please ensure that the target email set for mail forwarding is accurate, as this may affect senders trying to reach you . The Instructions on navigating to Mail Forwarding in Sheridan ‘s D2L email screen can be found here.
