{"id":23636,"date":"2023-07-26T09:58:38","date_gmt":"2023-07-26T09:58:38","guid":{"rendered":"https:\/\/ltsadev.sheridanc.on.ca\/apps\/wordpress\/learning-and-technology-portal\/?p=23636"},"modified":"2023-09-07T14:55:23","modified_gmt":"2023-09-07T14:55:23","slug":"adding-revising-forms-in-the-form-repository","status":"publish","type":"post","link":"https:\/\/ltsa.sheridancollege.ca\/digital-learning-support-hub\/adding-revising-forms-in-the-form-repository\/","title":{"rendered":"Adding \/ Revising Forms in the Form Repository"},"content":{"rendered":"<h3 class=\"p1\">What is a Form?<\/h3>\n<p class=\"p1\"><span class=\"s1\">A form template is a customizable, structured interface for users to enter information. \u00a0When you create a form template, you specify the appearance of the form and the types of information users can enter.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">When created in an individual course, forms do not copy between courses when using the Import \/ Export \/ Copy Components tool.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">There are two options available for publishing forms:\u00a0<\/span><\/p>\n<ul class=\"ul1\">\n<li class=\"li1\"><span class=\"s1\">You can export the form from one section, import it to another section and then make the form available to that course or,\u00a0<\/span><\/li>\n<li class=\"li1\"><span class=\"s1\">You can submit a request to move the form to the Organization level. \u00a0When forms are imported at the Form Repository the form becomes available to all SLATE users and you are provided with a link to the form that will not break when you copy between courses.\u00a0<\/span><\/li>\n<\/ul>\n<p class=\"p1\"><a href=\"https:\/\/ltsa.sheridancollege.ca\/digital-learning-support-hub\/exporting-and-importing-forms\/\"><span class=\"s1\"><b>Click here for instructions on manually exporting and importing forms.<\/b><\/span><\/a><span class=\"s2\"> If you are interested in moving forms to the Form Repository please\u00a0follow the instructions below on adding a new form to the repository.\u00a0<\/span><\/p>\n<h3 class=\"p1\"><\/h3>\n<h3>Adding a New Form to the Repository:<\/h3>\n<h4>Step One: Create the Form in a Course<\/h4>\n<ul>\n<li><a href=\"https:\/\/ltsa.sheridancollege.ca\/digital-learning-support-hub\/creating-a-form-template\/\">Instructions on creating a form template can be found here.<\/a><\/li>\n<\/ul>\n<h4>Step Two: Export the Form as a .zip package:<\/h4>\n<ul>\n<li>Within the course where the forms reside click on\u00a0<strong>Edit Course.\u00a0<\/strong><\/li>\n<li>Select\u00a0<strong>Forms\u00a0<\/strong>from the Course Administration page.<\/li>\n<li>Click on the checkboxes beside the forms you want to export.<\/li>\n<li>Click on the <strong>Export<\/strong>\u00a0<a href=\"https:\/\/ltsa.sheridancollege.ca\/learning-technology-portal\/wp-content\/uploads\/sites\/9\/2014\/09\/Export.png\"><img loading=\"lazy\" class=\"alignnone wp-image-8601\" src=\"https:\/\/ltsa.sheridancollege.ca\/learning-technology-portal\/wp-content\/uploads\/sites\/9\/2014\/09\/Export.png\" alt=\"Export\" width=\"26\" height=\"24\" \/><\/a>\u00a0icon.<\/li>\n<li>A pop-up window will appear. \u00a0When the export folder is ready you can click on the link to download the zip folder. \u00a0Save this folder to your computer.<\/li>\n<\/ul>\n<h3 class=\"p1\">If you are revising an existing form:<\/h3>\n<p class=\"p1\"><span class=\"s1\">Indicate this is a replacement of an existing form. Include the course name, section number and specific location where the form currently resides. \u00a0If the form resides in multiple courses \/ sections please include the name(s) of faculty that are teaching. \u00a0All faculty must be aware that the change is taking place as it will impact their individual sections. When sending the request please cc the other faculty members.\u00a0<\/span><\/p>\n<p class=\"p1\">Please note, prior to completing the below form please make sure you create a document that clearly outlines the changes to the existing form.<\/p>\n<h3>Request for adding\/revising a form in the repository:<\/h3>\n<p>Requests for new forms and form revisions must be received 3 weeks prior to the semester start date.<\/p>\n<p class=\"p1\">[si-contact-form form=&#8217;8&#8242;]<\/p>\n","protected":false},"excerpt":{"rendered":"<p>What is a Form?
