{"id":23605,"date":"2023-07-26T09:32:35","date_gmt":"2023-07-26T09:32:35","guid":{"rendered":"https:\/\/ltsadev.sheridanc.on.ca\/apps\/wordpress\/learning-and-technology-portal\/?p=23605"},"modified":"2023-09-07T15:00:08","modified_gmt":"2023-09-07T15:00:08","slug":"creating-a-form-template","status":"publish","type":"post","link":"https:\/\/ltsa.sheridancollege.ca\/digital-learning-support-hub\/creating-a-form-template\/","title":{"rendered":"Creating a Form Template"},"content":{"rendered":"<h4>Access the Forms Tool:<\/h4>\n<ul>\n<li>Select&nbsp;<a href=\"https:\/\/ltsa.sheridancollege.ca\/learning-technology-portal\/wp-content\/uploads\/sites\/9\/2013\/11\/Edit-Course.png\"><img loading=\"lazy\" class=\"alignnone wp-image-11818 \" src=\"https:\/\/ltsa.sheridancollege.ca\/learning-technology-portal\/wp-content\/uploads\/sites\/9\/2013\/11\/Edit-Course.png\" alt=\"Edit Course Icon\" width=\"105\" height=\"34\"><\/a>&nbsp;on the course navbar.<\/li>\n<li>From the Course Administration area select&nbsp;<a href=\"https:\/\/ltsa.sheridancollege.ca\/learning-technology-portal\/wp-content\/uploads\/sites\/9\/2014\/08\/form-icon.png\"><img loading=\"lazy\" class=\"alignnone wp-image-11876 size-full\" src=\"https:\/\/ltsa.sheridancollege.ca\/learning-technology-portal\/wp-content\/uploads\/sites\/9\/2014\/08\/form-icon.png\" alt=\"form icon\" width=\"98\" height=\"32\"><\/a><\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<h4>Create a New Template:<\/h4>\n<p><a href=\"https:\/\/ltsa.sheridancollege.ca\/learning-technology-portal\/wp-content\/uploads\/sites\/9\/2014\/07\/AddingFormTemplate.png\"><img loading=\"lazy\" class=\"alignright  wp-image-6544\" src=\"https:\/\/ltsa.sheridancollege.ca\/learning-technology-portal\/wp-content\/uploads\/sites\/9\/2014\/07\/AddingFormTemplate.png\" alt=\"AddingFormTemplate\" width=\"376\" height=\"238\"><\/a><\/p>\n<ul>\n<li>Under the Manage Form Templates heading click on&nbsp;<strong>New Form Template.<\/strong><\/li>\n<li>On the New Form Template page enter a name and description for the form.<\/li>\n<li>To make the form available to students, click the box under&nbsp;<strong>Make Form Available.&nbsp;<\/strong><\/li>\n<li>Click&nbsp;<strong>Save.&nbsp;<\/strong><\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<h2>Adding Fields<\/h2>\n<p>&nbsp;<\/p>\n<p>The content fields are the actual fields users fill out. &nbsp;There are two types of fields: system fields and custom fields.<\/p>\n<p>&nbsp;<\/p>\n<h4>System Fields:<\/h4>\n<p><img loading=\"lazy\" class=\"alignright  wp-image-6548\" src=\"https:\/\/ltsa.sheridancollege.ca\/learning-technology-portal\/wp-content\/uploads\/sites\/9\/2014\/07\/Add-System-Field.png\" alt=\"Add System Field\" width=\"314\" height=\"280\"><\/p>\n<p>System fields automatically populate with information from a users profile. &nbsp;For example:<\/p>\n<ul>\n<li>First Name<\/li>\n<li>Last Name<\/li>\n<li>Email Address<\/li>\n<\/ul>\n<h5>&nbsp;Adding a System Field:<\/h5>\n<ul>\n<li>On the Edit Form Template page select&nbsp;<strong>Add System Field.&nbsp;<\/strong><\/li>\n<li>A pop-up window will display. &nbsp;Select the checkboxes beside the System Fields you want to add to the form template.<\/li>\n<li>Click&nbsp;<strong>Add.&nbsp;<\/strong><\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<h4>Custom Fields:<\/h4>\n<p>Custom fields allow users to fill in content based on fields you define. There are several different options for creating fields including: drop-down lists, radio button list, check box list, text input &#8211; simple text, text input &#8211; formatted text, number input, date and time input, information, file upload, rubric evaluation and ePortfolio item link.<\/p>\n<p>&nbsp;<\/p>\n<h5>Adding a Custom Field:<\/h5>\n<p><a href=\"https:\/\/ltsa.sheridancollege.ca\/learning-technology-portal\/wp-content\/uploads\/sites\/9\/2014\/07\/Custom-Form.jpg\"><img loading=\"lazy\" class=\"alignright  wp-image-6637\" src=\"https:\/\/ltsa.sheridancollege.ca\/learning-technology-portal\/wp-content\/uploads\/sites\/9\/2014\/07\/Custom-Form.jpg\" alt=\"Custom Form\" width=\"311\" height=\"257\"><\/a><\/p>\n<ol>\n<li>On the Edit Form Template page click&nbsp;<strong>Add Custom Field<\/strong>.<\/li>\n<li>A pop-up window will display. Enter a&nbsp;Field Name.<\/li>\n<li>Select the&nbsp;Make this a required field&nbsp;check box if you want to make this field a requirement.<\/li>\n<li>Choose the&nbsp;Data Type&nbsp;for the field.<\/li>\n<li>Fill in additional information based on the data type.<\/li>\n<li>Click&nbsp;Save.<\/li>\n<\/ol>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<p><span style=\"line-height: 1.5em\">&nbsp;<\/span><\/p>\n","protected":false},"excerpt":{"rendered":"<p>Access the Forms Tool: Select&nbsp;&nbsp;on the course navbar.
